We need our students' parents' signatures for many different reasons. Below are examples of a field trip permission form, student information/ parent contact form, and photo permission form.
OR why not combine all the forms... at the bottom of your student information/ parent contact form, have an area for parents to approve photos and all field trips! Then you have only one form to file away!
